Our two campuses have board-certified nurses overseeing the health of our student body. The City Avenue campus also has a certified athletic trainer for students in grades 6-12. Students and parents are encouraged to contact one of our nurses with questions about managing illness, allergies, or medication at school.
We are members of the National Association of School Nurses and the Pennsylvania Association of School Nurses and Practitioners. Lower Merion School District provides part-time nursing assistance for the implementation of our state-mandated programs, including immunization surveillance and student health screening.
Information about Student Health
We will be launching a new portal in August 2021 where you will be able to upload all required medical forms in one place. Stay tuned for more details. A School Physical Examination Form is required for every student every year. It is due on the month of your child's birthday or on the anniversary of your child's last physical examination. Participation in 6th through 12th grade athletic activities will be denied if a current Physical Form is not on file.
A Medication Request Form and a Physician Order for prescription medication is required for any student who needs to have prescription medication (daily medication, EpiPen, inhaler, insulin, etc.) kept at school. The original pharmacy-labeled container is acceptable in lieu of a written prescription.
A Dental Examination Form is required for all students in Kindergarten, grade 3, and grade 7.
Submitting Your Child(ren)'s Forms
Download, print, and complete your child(ren)'s forms, then scan/photograph and email them to firstname.lastname@example.org. We will be launching a new portal in August where you will be able to upload all required medical forms in one place. Stay tuned for more details.
